About Chronic Disease

Chronic disease management requires ongoing care and support to assist anyone impacted by a chronic health condition with the medical care, knowledge, skills and resources needed to better manage on a day-to-day basis.

Chronic disease prevention focuses on lifestyle changes—including healthy eating, regular physical activity, stress management, and avoiding tobacco or excessive alcohol—to stop conditions like heart disease, diabetes, and certain cancers before they develop. By addressing risk factors such as poor diet and inactivity, up to 80% of these conditions can be prevented.
